E-Surveys

While the Enhancing Interdisciplinary Collaboration in Primary Health Care (EICP) Initiative will be conducting formal consultation sessions with health care providers, opinion leaders and patients, it also wants to keep the door open to the opinions of all Canadians with a viewpoint about collaborative care.  As each wave of research and consultations proceeds, a new on-line survey will be posted to the EICP web site as a companion to the on-site consultation sessions.  If you register with our Guest Book, we will let you know when a new survey has been posted.  Results of these surveys will be analyzed and posted regularly to the web site.
